The most recent chapter in the history of PTH is its use as the most effective current therapy for osteoporosis. This fact is quite surprising and clearly paradoxical

the paradox was merely the difference between results with continuous elevation of PTH (i.e. bone loss) and intermittent, short elevations of PTH (i.e. bone mass increase)

Intermittent high PTH is anabolic for bone, while continuous elevation is catabolic

The only receptor for PTH is on the osteoblast

The beneficial effects of PTH are primarily in trabecular bone
